Why do I have duplicate SQL databases or logical disks in the console after a version upgrade?

    Kevin - How might such a situation work if we did an in-place OS upgrade from 2012 R2 to 2016 on our Management Servers? In fact, on the very first two (2) Management Servers we ever had, we started on 2008 R2 then to 2012 R2 and now 2016. I have monitors running for all three OS's. I have scoured the web looking for solutions but could not find anything.
