Konstruktor ProxyAccount (JobServer, String, String, Boolean, String)

Inicjuje nowe wystąpienie ProxyAccount klasy na określonym SQL Server Usługi spedycyjnej z określoną nazwą i poświadczeń. Określa również, czy włączono konto proxy.
