IVsMenuEditorSite.CreateItem Method

The host is responsible for giving the menu a unique name, and providing back to IVsMenuEditor a valid IVsMenuItem*. As in IVsMenuEditor::AddItem, both IMIParent and pIMIInsertAfter can be NULL.
