    The first and second questions on the survey (person name and company name) appear to be misconfigured; the form validation is asking that a valid email address be entered in both of those fields and is preventing the form from being submitted.

      The same for me, but it's maybe my mail.... in this case, I would be sorry to see that I can't register with an email coming from an organisation with a mail such as .org
