Alan Page on Root Cause Analysis

I have worked with Alan off and on for a very long time now.  When I heard that he gave a Lightning Talk at STAR East, the first thing that came to mind was "I wish I could have seen that".  Last week, he posted what he talked about (and more?).  I very much recommend giving his "Bug prevention in five minutes" post a read.  He presents a great technique for root cause analysis (RCA) -- including one of my favorite analyses (I don't want to be a spoiler here, but it involves lack of sleep).
