Two comments that I frequently see for this popular absence request form are requests to add enough room to track multiple employees and to design it for digital signatures, especially for companies with employees in more than one location.

If that's what you need, try the new absence request form (for employees) from the community templates. It allows for more than one employee's information and for digital signatures. With holidays and the new year approaching, you can also find more absence request and notification templates, both for employees and for students. There are versions for more than one program, including Word, InfoPath, and Excel.

Don't forget that if you don't see a template that meets your needs,  you can always submit a community template to share what works well for your business or personal use.
