Why does my Authenticator App say "action required" on my accounts even after I've backed them up?

I got a new phone, and redownloaded Authenticator, backed it up, etc. Now the accounts say "action required", and I cannot use the app. Whenever I click on the account, it asks for a QR code or a number code. I have no access to these, because the accounts are for college portals, and they only show you the QR code once. What do I do?
