After installing KB 5044062 on Exchange 2019, my mail flow rules stopped working
After installing the above, I used HealtChecker and it asked to change the transport service to the following:
Set-TransportService Mailbox1 -MaxPerDomainOutboundConnections 40 (before change 20)
Set-TransportService Mailbox1 -MessageRetryInterval 00:05:00 (before change 15)
If I run the following:
Get-TransportRule -Identity "Spam" | Format-List Name,Priority,State
I receive a message that the rule is enabled.
I need a hint :-)

At the moment, Microsoft has listed it as a known issue and is trying to fix it permanently. As an alternative, you can try re-enabling the service or uninstalling the update to ensure that the mail flow is working properly.

Complete the following steps to complete your CU deployment.
Put the Exchange server in maintenance mode, and then reboot the server.
Install the latest applicable security update (SU) as listed in Exchange Server build numbers and release dates. Follow these steps:
Select Start, and then enter cmd.
In the results, right-click Command prompt, and then select Run as administrator. If the Account Control box appears, verify that the default action is the action that you want, and then select Continue. Enter the full path of the .msp file, and then press Enter. __ When installing the security update manually you must run it from an elevated command prompt. If you don't, the install might complete, but the server won't be protected. Reboot the server, even if you aren't prompted to do so. Check that all Exchange services are back to their previously noted state. Take the server out of maintenance mode. Check all additional applications that connect to Exchange, including your backup, archiving, and monitoring solutions. -

2024-11-14T11:43:15.9166667+00:00 I have a similar problem with transport rules failing to fire after KB5044062, but I'm still on 2016.
You wouldn't happen to also be running ESET Mail Security would you?
I have gotten the rules working again (for a little while now anyway) by disabling mail transport protection and restarting the exchange transport service.-

I use the Zyxel ATP 500 firewall which has mail security and Exchange 2019's own filters where I have also created some rules in Mail Flow.

Rename the rule to _old for example, and clone it to the original name.
Save the rule, put the rule at the top (i've done this, but maybe it's not necessary) and do not change it anymore.
It worked for me
