The "Provision failed" status indicates that the VM agent is not installed or not running properly.
Suggest you try the following steps:
- Check if the VM agent is installed on the source VM. You can do this by connecting to the VM and checking if the "Windows Azure VM Agent" service is running. If it is not running, try starting the service manually.
If the VM agent is not installed, you can install it manually by following the steps in the Azure documentation: https://docs.microsoft.com/en-us/azure/virtual-machines/extensions/vmaccess-windows
If the VM agent is installed but still not running, you can try restarting the VM to see if that resolves the issue.
