Excel 365 crashes after Macro to create a new workbook

The organization runs a workbook with macros that will create a new workbook from a template we created. After a VBA subroutine using a form for the end user to enter the desired file name and testing the characters of the name (to omit slashes, and other problematic characters), the new file is created. Recently, on one computer, after about 3 times working OK, the file gets created and Excel crashes. It sometimes reopens the files in recovery mode.

The only notification in the Events shows a problem with FM20.dll and OFFICE_MODULE_VERSION_MISMATCH

I can't find any fm20.dll file.

I've run the Office 360 Repair, the Office enterprise repair and the Office business repair.

SFC only found and repaired 3 files-BthA2db.sys, BthHfEnum.sys and bthmodem.sys.

Running Windows 11 Home 23H2

Most recent changes were Definition Updates KB2267602 ver 1.421.1336.0

    Hi,

    "OFFICE_MODULE_VERSION_MISMATCH" this error may be caused by an Office module version mismatch, try these methods:

    1. Make sure all Office applications are up to date.
    2. Run the Office Repair Tool to ensure that all modules and library files are installed and updated correctly.

    FM20.dll files are usually located in the C:/Windows/syswow64/ directory. If you can't find this file, you can try reinstalling Microsoft Forms 2.0 Object Library, or you can do an online repair for Office:
