How can we add short custom surveys that pop up in a SharePoint site?

Our organization is looking to collect some data about our employees' experiences with our SharePoint intranet site. It doesn't appear that there are out of the box options for quick feedback on the site, like a box popping up when an employee clicks on something that says "Did you find what you were looking for?" or "How would you rate your site experience?" Is there a custom script workaround that could accomplish this?
