What is causing "Accept permissions failed" in Deployment step of deploying integrated app?

I'm trying to deploy an integrated app through the Microsoft 365 admin center. I get to the Deployment/Permissions step and login with my account, but I never get the chance to accept permissions. Then I get the error "Accept permissions failed" when I try. I am logged in as the global administrator. Can anyone tell me how to get past this or where I can find the logs for this?
