How to Use Select state with vba listbox

Question

I have a userform with a list box and some controls on it.I have write a code with in the list box in such a way that when i click any item of the listbox,the caption change according to the selection of listbox.For this purpose i have used "Select Case".But the code is working for the first two items and after that the code is not working for other items.Please help me to correct my code.The file is in attachment in link.
