Task not getting created in SLA in CRM 2015

We had created a new enhanced SLA which had an SLA item having an email in Success Actions and a task in Failure Actions.

 

 

 SLA Item:

 

Neither the email was sent nor the task was getting created.

Solution:

Select Resolve By KPI in SLA KPI

Default value selected in SLA KPI is First Response by KPI

Changing SLA KPI from First Response by KPI to Resolve By KPI solved the issue
